This position implements the protection and promotion of the health of Chisago County residents through promotion and provision of population based public health services.  Works as a public health division liaison to community partners, the public at large, public health systems and other County partners. Carries primary responsibility for the development and delivery of Statewide Health Improvement Program (SHIP) in Chisago County, following MDH strategies directed at population based health deliveries.  Work includes planning, executing, and evaluating evidence-based community health strategies in target areas as identified by SHIP. Hours of work are Monday to Friday 8:00 a.m. to 4:30 p.m.


A Bachelor’s degree or higher from an accredited college or university in Public Health, Community Health Education, Nursing with a health education focus, Nutrition and Food science, Health Policy, Community or Organizational Development with a Health Focus curriculum and 2-3 years of experience.  $21.64 to 29.65/hr DOQ.

  1. Coordinates efforts to provide population based public health initiatives with internal and external partners and acts as community health practice liaison                                  10%
    1. Research best practices and evidenced based models for broad population health.
    2. Develop, initiate, and evaluate community health approaches and strategies as identified in our community health assessment and improvement plan
    3. Create and deliver health messaging to targeted populations for the purpose of development of health in all policies and the improvement of the population’s health in Chisago County.
    4. Pursues and evaluates promising population health practices, with the CHSA, determines the application of same to the citizens and communities of Chisago County. 
    5. Acts as a community resource for assuring public health best practice 
    6. Seeks grant opportunities and assists with writing, reports, management, and evaluation of grants
    7. Prepares for and delivers health initiative presentations to a wide range of interested others.  
    8. May assist with developing and marketing  for existing or new community based health related programs

 

  1. Provides guidance on population health, following the ten essential public health services  and public health’s core functions                                                                                   5%
    1. Contributes to the community health assessment, planning, implementation, and evaluation
    2. Part of the community health assessment team to identify community priority goals and objectives
    3. Assists in developing the community health improvement plan and assures implementation follows the established community health plan
    4. Provides facilitation for the evaluation of the community health plan and makes recommendations for modifications as needed  

 

  1. Assess, implement, and evaluate continuous quality improvement measures of population health                                                                                                                            10%
    1. Active member of quality improvement team and provides coordination for quality improvement team
    2. Assist and identify targeted populations for health inequities and disparities
    3. Plans, partners and initiates community responses to identified inequities
    4. Coordinates a systematic process for determining efficient and effective impact of CQI measures and outcomes

 

  1. Coordinates the  Statewide Health Improvement Program Initiatives (SHIP)            70%
    1. Oversees assessment of school policy, systems and environments related to nutrition and physical activity as required for SHIP grant.
    2. Provides technical assistance, resources, and training to aid schools in wellness policy development, implementation, evaluation and compliance with federal guidelines for nutrition and physical activity.
    3. Provide technical assistance and resources for other health promotion initiatives as assigned by CHSA.
  1. Under the direction and with the CHSA, works to identify, promote and delivery population based health strategies in a variety of settings in Chisago County with a variety of populations.
  2. Assures county delivery of Statewide Health Improvement Grant as outlined in the Minnesota Department of Health Grant Agreement. 
  3. Monitor and report results and outcomes of a policy or program.
  4. Demonstrates ability to satisfactorily work with SHIP software for Minnesota Department of Health reporting purposes and county department software required for time reporting and other internal reporting.  
  5. With the CHSA engages the community through public presentations, meetings with organizational leaders, residents, and others to promote health in all policies.
  6. Engage community partnership to build the community's health promotion capacity and strengthen systems that support community health and health policy. Identifies gaps in the community for partnerships to address a health needs.
  7. Is a contributor to the community assessment to determine local health system's strengths, gaps, and opportunities. 
  1. Develop educational and promotional material, reports, plans, grant applications, and related communication tools to support health policies and programs.
    l.  Represent the department and county to the public, internal and external groups and organizations. Develop and present educational material.
  2. Educates others on the focus of policy, systems, and environment
  3. Under the direction of the CHSA and counsel if applicable, oversees, and monitors the work of contractors to the program, if present assuring adherence to contract
  4. Oversees and recommends the approval of mini-grants as grants are available following program guidelines related to the funding entity.
  5. Provides mentorship to staff regarding community health initiatives

 

  1. Miscellaneous                                                                                                        5%
    1.  Participates with public health emergency response functions as trained and assigned within the department’s emergency response plan.
    2. Other duties as assigned
    3. Participates in training as assigned
    4. Demonstrates adherence to miscellaneous County and HHS Department policies.
    5. Keeps and Maintains Automobile Insurance and valid Driver’s License at all times.
    6. Demonstrates adherence to and knowledge of HIPAA and Data Practices in work assignment.

 

     VI.        Education and Experience

                  a.   Graduation from High School or equivalent, including the following;

                  b.   Minimum Qualifications

            Bachelor’s degree or higher from an accredited college or university in Public Health, Community Health Education, Nursing with health education focus, Nutrition and Food science, Health Policy, Community or Organizational Development with a Health Focus curriculum, Other health related degree. 2-3 years of experience.   Must have a valid driver’s license and car insurance at all times.

Preferred Qualifications

            Master's degree in Public Health or related Health care or education focus

VII.     Knowledge, skills and abilities.

                  a.   Knowledge of nutrition and physical activity principles, policies, and practices.

      b.   Knowledge of ten essential services of public health and six core functions.  Ability to apply principles of public health to communities, families, and individuals.  

                  c.   General knowledge of health promotion (population and community-based

                        primary prevention).

                 d.    Knowledge of community organization systems, norms, and resources

     e.    Knowledge of educational, training and consultation principles, practices, materials, and tools with various populations.

                 f.     Ability to speak and present publicly.

                 g.    Ability to establish effective working relationships with staff,

                       community, and professional groups.

                 h.    Ability to exercise tact, judgment, initiative.

     i.     Ability to work independently and set work priorities complimentary to the work of the Department.

                 j.     Effective communication skills, both oral and written.

    k.    Employee must have demonstrated ability to work effectively and efficiently in a wide variety of computer programs and systems.

     l.     Ability to establish and maintain effective, respectful, relationships with clients, agency staff members, and the general public.

    m.    Ability to apply previously acquired knowledge and skills to new situations.

    n.     Ability to communicate effectively using a variety of methods.

    o.     Must be able to maintain positive working relationship with County Attorney, County Administration, Department Administrative team, and other county and community committees, State offices of the State of Minnesota for miscellaneous programs.
